SCRT have number of exciting new vacancies for a fantastic career development opportunity.
This is a newly created Home from Hospital team which covers the West Lothian service area and has been set up as a fast response team to enable service users who are ready for discharge from hospital to safely return to their own homes.
Clients will have a care package in place and you will be required to work with service users over a period of time to establish ongoing care requirements for support at home and to safely transition clients to mainstream care at home providers. Predominantly you will be covering Livingston and Broxburn and you will have access to your own transport.
